Nastavení modemu Zyxel na ČRa

Nemohl by mě tu prosím někdo napsat nastavení modemu Zyxel 650R na České radiokomunikace. To co mě poslaly je na ... a to co jsem našel sice funguje ale neustále to vypadává. Nevím v čem je problem. Děkuji
